First off you should really rule out medical problems like resting to make sure it ain't a stress injury or one abut to happen and if it doesn't work perhaps seeing a doctor. Try starting slower with the arm pull from the reach back keeping arm muscles totally loose. Then whipping the elbow straight while turning the shoulders fast allowing a much larger follow through arc with the arm due to the increased speed.
Flat shots need running on the center line of the tee and planting each step on the center line. Anhyzer needs running from rear right to front left with the plant step hitting the ground to the left of the line you're running on. Hyzer is the mirror of that.
