1. your head is forward but not your center of mass (which is just above your solar plexus or your heart.)
2. that thing you do where you point the disc nose up at your target during your run-up is making you throw nose up.
3. you are still strong arming all your shots.

practice throwing smooth easy shots with a putter or a fastback. and practice throwing them low and flat. try pulling across your navel.

when you start throwing weight forward you will stop falling backwards after your shot (which you do a lot). when you finish your shot you should actually take a step towards your target.

Stop everything you're trying to change until you've managed to do this: Kick hard with the left leg back to front initially. There are more tricks that you can do later but lets start with the basics. The goal is to have the left leg straight with no knee bend at the rip. The right knee is ok so keep the same angle and really push yourself forward with that left leg. It is the largest power generator of single body parts and you're not using it at all!
Flat shots need running on the center line of the tee and planting each step on the center line. Anhyzer needs running from rear right to front left with the plant step hitting the ground to the left of the line you're running on. Hyzer is the mirror of that.

I would suggest realy leaning into the throw alot more. Your entire upper body should not be so vertical. look at these pics for example : the shoulders are in front of the right foot from all the energy that pushes them forward. http://site.just-disc-golf.com/yahoo_si ... 10_std.jpg http://www.discgolfassoc.com/education/ ... lay640.jpg This also keeps your disc low.

And yes, pushing with your left leg. This will also help the leaning forward.
