A little tough to see from the angle, but it looks like you are not getting your disc into the power area. Your arm appears to stay behind your body through the entire throw. Try to hold off on the body/shoulder rotation and pull your elbow through like you are trying to elbow someone in the chest. Then when your body turns, you can release the elbow joint and your lower arm will whip out.

Flat shots need running on the center line of the tee and planting each step on the center line. Anhyzer needs running from rear right to front left with the plant step hitting the ground to the left of the line you're running on. Hyzer is the mirror of that.
