Has anyone ever actually called a courtesy violation? At what point?

I've seen a guy unload about 15 discs in all directions after a missed short putt.....I laughed....very, very hard.

I got so pissed the other day i threw one of my wizards at a picnic bench and it whizzed by and went right in the pond , im usually pretty well mannered on the course too

i've been warned before, but no one took it seriously. the guy who warned me had been banging his head against a tree a few holes before and yelling curses all round long, so when he "warned" me everyone on my card laughed.
