I have read a lot about disc pivot and wrist extension. I was just wondering how closely the two are related. Is wrist extension simply the way one goes about getting good disc pivot? Or do I just not fully understand these terms.

The way I interpret those terms is that wrist extension describes the actual movement of your wrist during a good "hit." Disc pivot describes the movement of the disc during a good "hit." You get disc pivot if you time wrist extension correctly.
