I have some shoulder problems too. Often the rear shoulder muscles are under-developed. There are some exercises you can do with those stretchy bands that really target the rear shoulders. I'll try to explain the best I can...you may want to do some surfing to find pics.
Fix one end of the band to something about waist high.
Exersize 1:
Facing the fixed end hold the other end in your hand
Keep your elbow at your side at all times, and bent at 90 degrees
Start with your hand pointing toward the fixed end and move it rearward...kind of the reverse of throwing a sidearm.
You should stand far enough from the fixed end that there is constant tension on the band, but you should still be able to do about 20 reps.
Exersize 2:
Same here, but raise your elbow so that your upper arm is parallel to the ground.
Start with your forearm pointing at the fixed end and rotate it to pointing at the sky. Really concentrate on feeling the rear shoulder muscles doing all the work.
Exersize 3:
Reverse of Exersize 1. This one actually uses the front of the shoulder, but still uses often underdeveloped muscles that surround the rotator cuff.
This one you want to face 90 degrees away from the fixed end.
Keep your elbow at your side again
Start off pointing at the fixed end and rotate your arm bringing your hand to your stomach.
You may want to double up the band for this one as this is a naturally stronger motion.
