Looks like you're kinda shooting from the hip, holding the disc low during the reachback. All these reps look like big hyzers or hyzer flips. Experiment with holding the disc higher during the reachback.
You're aiming with you're shoulder, which is good, but you should aim with your elbow too. Raise it up and point it towards the basket or the line you want to hit. Then concentrate on pulling your elbow through at that exact angle during your toss.
You're hurrying your throws and going through the reps too fast. Relax and make a smooth, controlled toss.
