1. your head is forward but not your center of mass (which is just above your solar plexus or your heart.)
2. that thing you do where you point the disc nose up at your target during your run-up is making you throw nose up.
3. you are still strong arming all your shots.
practice throwing smooth easy shots with a putter or a fastback. and practice throwing them low and flat. try pulling across your navel.
when you start throwing weight forward you will stop falling backwards after your shot (which you do a lot). when you finish your shot you should actually take a step towards your target.
