Two things really jump out with this. Your weight is waaaay back. Slow down a bunch and take a much smaller final step. Pull later so your momentum from the runup carries you forward, instead of your arm pulling you backwards.
Your footwork looks off too. You hardly get the first full step down, it looks like your about to fall over. You can see your leg get really limp, i'm not sure how you keep yourself up. I think slowing down and taking smaller steps will help that as well. Once you get your balance together and slow down your steps, post another video. Its going to change things alot.
